

How Do I Get Started?
Research bariatric surgery and determine if it is something that you really want to do. Obesity surgery is not an "easy way out" or a "quick fix". It is a major endeavor that helps most people but has resulted in death and unpleasant experiences for a few.
Find out what your body mass index (BMI) is and determine if you are a candidate for surgery. You can do this by reviewing the "Do I Qualify" section of this site.
If you are a candidate, decide on which procedure you desire. You may change your mind about which procedure you desire after your initial surgical consultation. This is normal. You an learn more about your options at The American Society for Metabolic and Bariatric Surgery.
Contact your insurance company representative to check if you have benefits for bariatric surgery and which procedures are covered. Some insurance companies have limits on bariatric coverage. If so, full prepayment is required prior to scheduling surgery.
Down load and print the medical history form below or call our office to have one mailed to you. Fill our the form and mail it to the address on the the last page of the form.
After we have reviewed your medical history form we will contact you regarding an appointment. If you are not contacted within 7 - 10 days after mailing your medical history form, please call our office at (330) 344-1100.
